Fix JaCoCo startup crash on Android by setting output=none via ContentProvider (#469)

JaCoCo offline instrumentation causes ExceptionInInitializerError on startup
because the agent tries to write /jacoco.exec to the read-only root filesystem.
Add a JacocoInitProvider ContentProvider (in the coverage package, excluded from
instrumentation) that sets the jacoco-agent.properties system property to
output=none before any instrumented Activity class loads. Coverage data collection
is handled separately by CoverageBroadcastReceiver via reflection.
