fix: port three post-M145 upstream fixes

1. AEC3 filter switching: decide refined vs coarse filter once across
   all channels instead of per-channel, preventing inconsistent filter
   selection in multi-channel configurations (upstream 297352a).

2. Comfort noise generator: pre-compute random sin-table indices once
   and reuse them identically across all channels, ensuring consistent
   comfort noise in multi-channel mode (upstream 573e746).

3. Pipeline config defaults: change multi_channel_render and
   multi_channel_capture defaults from false to true, matching upstream
   behavior change (upstream 42e location TBD).

Also ports the BasicEchoRemoval integration test from
echo_remover_unittest.cc, verifying echo attenuation across channel
counts (1, 2, 4), sample rates (16k, 32k, 48k), and delays
(0, 64, 150, 200, 301 samples).
