fix(notify): address Copilot review findings on PR #71

- use context.Background() for success notification (parent ctx may be canceled)
- capture stdout in custom script executor error messages
- comment out notify_on_error/notify_on_complete in defaults config
- add code-level defaults for notify flags when not explicitly set
