Migrate to `env_bootstrapper` and `app_starter` (#35)

*   Make all entry scripts use either `env_bootstrapper` or `app_starter`.
*   Remove obsolete `run_main` and `switch_to_venv`.
*   Rename `PythonExecutable` to `StateStride` (and remaining functions).
*   Reorder `state_input_py_exec_var_loaded` to be the first.
*   Remove `state_app_starter_invoked` (replaced by `_start_main`).
*   Rename `EnvVar.var_PROTOPRIMER_TEST_MODE` to `EnvVar.var_PROTOPRIMER_MOCKED_RESTART`.
*   Update docs and `readme.md`.
