Improve documentation when exceptions are thrown (fixes #81) (#87)

- Many of our public functions did not explain properly the cases where
exceptions could be thrown, leading to unexpected crashes. This PR
improves documentation to prevent such issues.
- This PR also reduces the number of throwing functions/cases where it
was not absolutely necessary.
- Some functions that could only be used with exception handling now
allow avoiding that by providing ways to check whether `xlnt::optional`
members or other states are set, or conditions are met.
- Many exceptions have improved error messages.
- Unit test debugging is now easier with improved exception messages.
- Documentation has been improved.
