Add PR size guidelines to prevent oversized PRs (#3482)

Add PR size guidelines and split-pr skill

Add comprehensive PR size guidelines to CLAUDE.md that require Claude to
check PR size before creation and propose splitting when changes exceed
reviewability limits (400 LOC, 10 files). This ensures PRs remain focused,
reviewable, and atomic.

Also add /split-pr skill that analyzes current changes and suggests how to
split large changesets into logical, independently-testable PRs. The skill
provides structured analysis of dependencies and proposes creation order.
