Add per-type container routing for Cosmos DB document store (#1029)

* Initial plan

* Implement container routing in AzureCosmosDocumentStore

- Add container_routing_mode parameter (legacy/per_type)
- Update schema and regenerate typed configuration
- Implement helper methods for container routing
- Update all CRUD operations to use routing logic
- Support both legacy single-container and per-type multi-container modes

- All existing tests still passing (62/63, 1 unrelated failure)
