Fix MOH 2020 romanization mapping and rules

- Change ຍ from 'ny' to 'y'
- Change ວ base mapping from 'w' to 'v'
- Add context-aware rules:
  - Silent ຫ before v/y/l/r/n/m
  - Final 'd' -> 't' in multi-char syllables
  - Final 'v' -> 'o' in multi-char syllables
  - Vowel 'e' reordering (ເ placement)
- All tests now passing
