More work to fix preview deployments (#104)

* Dummy commit

* Added master build caching to speed up initial PR builds that didn't touch Docker content, fixed some issues with preview-deploy

* Fixed owner/maintainer check for PR author to auto-approve deployments Dave Young creates, fixed preview to ensure the base ephemeral stack exists before anything

* Fixed preview to select specific stack

* Updated install.sh to get rid of things now managed by Pulumi, updated README.md to reflect all the changes we've made, added some more cleanup tasks for README

* Fixed preview environment protections

* Added auto-approval for admins to preview

* Fixed race condition in auto-approval

* Fixed string-integer coercion for environment ID

* Fixed preview deploy to base64 decode KUBECONFIG secret before using it

* Fixed missing doctl install on preview deploy

* Fixed PostgreSQL connections, clarified spec and CLAUDE.md about using hacky workarounds (Claude recommended port forwarding, which was not the right approach)

* Removed hacky fallbacks in Pulumi, added better error handling around posting comment with Pulumi preview

* Fixed detecting if stack does not exist

* Fixed some preview deployment stuff, removed out of date file structure ascii art, simplified stacks so that ephemeral is not in its own directory and follows the rest, also renamed dev-local to just local for simplicity

* Updated the plan - we are now using separate clusters for preview vs production because things were getting too complicated with the shared dependencies between preview and production.  Next is implementation
