refactor: Modularize CLI and chunkers, fix destination logic, update docs

- Split CLI chunk command into helpers (_extract_files, _write_chunks, _print_chunks) for better modularity and readability.
- Refactored PlainTextChunker with _prepare_next_chunk, _validate_constraints, and improved state handling.
- Refactored CodeChunker with _group_by_chunk and enhanced method organization.
- Fixed CLI destination handling to consistently support JSON files (.json extensions) and directories, removing input count restrictions.
- Updated cli.md and code_chunker.md to clarify destination behavior, JSON output, and add new scenarios.
