Change how TranscriptVariations are cached on VFs

Current TranscriptVariations are cached on a VariationFeature keyed
on the transcript's stable_id. This is not guaranteed to be unique
in a couple of situations (otherfeatures DB, VEP using custom GFFs),
so now the key is compiled from the stable_id and dbID.
