fix(token): fix lazy() methods returning same instance instead of new one

Fixed bug in SingleAliasToken and GroupAliasToken where lazy() was returning
the same instance instead of creating a new instance with the lazy flag set.
This prevented proper lazy loading behavior.

Also improved documentation:
- Added comprehensive select utility documentation (token, alias, instances, scope.current, scope.create)
- Documented 4 missing token types (ClassToken, FunctionToken, ConstantToken, GroupInstanceToken)
- Updated token hierarchy diagram to show all 8 token types
