feat: Match Phoenix backlog format with summary header and sections

- Add summary header with Total Requirements, MISSING/PARTIAL counts, Estimated Effort
- Show Critical Path and Quick Wins sections together in default view
- Use "Phase X" format for phases
- Add Effort column to tables
- Format blocks as "X reqs" or "X (Y)" showing transitive (direct)
- Add "Status" column header
- Use triangle (△) for PARTIAL status to match Phoenix
