Replace rspec-rails with Rack::Test

The main difference is that Rack::Test uses the name
last_response as opposed to Rails response.

This also includes a small bugfix, the HealthController
was previously consulting the wrong Header to determine
if it should display JSON (Content-Type should have
been Accept).

Closes #409
