docs: fact-check format crate matrix against actual tests

- Fix dead links in docs index (format-crate-matrix, extension-attributes)
- Add facet-xml to the format matrix
- Fact-check all 12 format crates against their actual test suites
- Add HTML title attributes with test names for verified features
- Remove duplicate internal format-test-matrix.md (consolidated into public docs)
- Fix parser info: toml uses toml_parser, xml uses quick-xml, urlencoded uses form_urlencoded

Key corrections:
- json char: ✅→🚫 (no tests), u128/i128: 🟡→✅ (tested)
- yaml u128/i128: 🟡→✅ (tested)
- msgpack f32/f64: ✅→🚫 (deser not implemented), sets: ✅→🚫
- toml sets: 🟡→🚫, smart pointers: ✅→🚫 (no tests)
- xdr Option: 🟡→✅ (tested), sets/maps: 🟡→🚫
- urlencoded: most types downgraded (only String/u64 tested)
- csv Option/NonZero: 🟡→✅, Vec/arrays: 🟡→🚫
