refactor: Consolidate PR fetching into one hook (#130)

* refactor: Consolidate PR fetching into one hook

The dedicated hook `useAllPullRequests` has been removed.

Functionality for fetching all pull request pages (up to the 1000 result
limit) is moved into `usePullRequests` via a new `fetchAll: boolean`
option. This simplifies hook management and reduces duplication.

The generic hook now also accepts `refetchInBackground` options.

Fixes #115
