refactor(storage): create sqlite database path only when creating connections (#426)

Move ensure_cache_dict() from __init__ to _ensure_connection() so cache
directories are only created when actually establishing a connection,
not when using nondefault connections.

Added tests verifying directory creation is skipped with custom connections.
