Calculate Content-Length to avoid chunked encoding.
* Improve detection for fs.createReadStream's `autoClose` option
* SuperTest (which uses Formidable) doesn't seem to be able to correctly
  parse multipart/byteranges bodies so I've come up with a little hack.
