#71: Update license to MIT + Commons Clause + Attribution

- Updated LICENSE file from MIT to MIT with Commons Clause and Attribution
- Added Commons Clause to prevent selling MSR or database adapters as products
- Added Attribution requirement for derivative works and extensions
- Created NOTICE file with quick FAQ about license usage
- Created comprehensive /docs/license.md with detailed examples and scenarios
- Updated package.json license field to "SEE LICENSE IN LICENSE"
- Updated README.md with license summary and TL;DR
- Updated home page (/docs/index.md) with license section and FAQ link
- Updated license badges in README.md and home page to "MIT + CC + Attribution"

License allows:
- Free use in commercial applications
- Forking and contributions
- Creating free database adapters (with attribution)

License prohibits:
- Selling MSR or adapters as standalone products
- "MSR-as-a-Service" for money
- Paid consulting focused specifically on MSR

Note: This license applies to v0.3.0 and later. Previous versions (v0.2.2 and earlier) remain under MIT license
