Unify URLS for `ibm_cloud` and `ibm_quantum_platform` channels (#2486)

* Update URL resolver to stop generating decomissioned URLs when channel is set to ibm_cloud. Instead of relying on redirects, now both ibm_quantum_platform and ibm_cloud channels use the same URLs to access the runtime entrypoints (they were already using the same URLs for authentication). Also updated docs that pointed to the wrong base API value and added examples to clarify where and how each URL is generated.
