Increase test coverage >95% (#22)

Increase test coverage to cover current API. Add logic
to `logger.js` to respect given log limits. Add npm
coverage script and add configuration in .travis.yml to
export the stub credentials file used for testing client
configuration. Add coverage to `.gitignore` to ignore
generated files from added npm coverage script.

Dependencies Added:
`mock-fs` - Added to mock logging so that log statements can be
  analyzed for formatting correctness.

Respond to pr feedback by removing forgotten `console.log` call.
Node 6.4.0 is introducing some unexpected output for newlines in
util.inspect by producing \r\n. This has been normalized in the
test since it is a newline, but this is the only function exhibiting
this behavior as of now and it seems amiss. Test was updated
accordingly with documentation.

Respond to pr feedback - adjust authorizedRequestFactory tests
to only use a `.once` instance of nock for single request tests.
