fix: resolve race conditions in output capture and job event handling

address two production race conditions and one test artifact:

- add mutex to OutputCapture for thread-safe concurrent writes from
  stdout/stderr pipes during command execution
- move database operations outside jobsMu lock in handleJobEvent to
  prevent blocking dashboard reads during persistence operations
- add safeWriter test helper to fix race in integration tests where
  multiple jobs write to shared buffer

verified with go test -race across full test suite.
