Refactor: Replace all checkboxes with ch-checkbox web component

- Implement SVG checkmark/dash icon overlay on checkbox input
- Apply blue-700 (rgb(29 78 216)) styling for checked/focus/indeterminate states
- Update header-row, popup template, and enable toggle to use ch-checkbox
- Remove native HTML checkbox styling in favor of unified web component
- Update all test files to use CheckboxElement type (zero any types)
- Add side-effect import for component registration + type import pattern

All 304 tests passing with full type safety.
