fix: improve Coveralls integration for proper source file mapping

- Add fetch-depth: 0 to checkout for full git history
- Add verification steps for source structure and coverage report
- Use 'files' parameter instead of 'file' for Coveralls action
- Add flag-name and parallel configuration for proper job identification
- Create .coveralls.yml configuration for explicit source file mapping
- Add debug mode to Coveralls action for troubleshooting
- Include coverage report validation to show file references

This fixes the 'source not available' issue on Coveralls by ensuring
the LCOV report contains proper file path references and Coveralls
can locate and display the source files.
