Add configurable metadata_timeout option for long-running streams (#115)

Metadata collection can timeout on large documents (e.g., 91K characters)
since the metadata task can't complete until the stream is fully consumed.
The previous hardcoded 60-second timeout was insufficient for these cases.

This change:
- Adds :metadata_timeout option to start_stream/4 (default: 60_000ms)
- Respects application config :metadata_timeout if set
- Allows per-request override for long-running streams
