Rename Bedrock to AmazonBedrock and address all review feedback

- Rename all modules from Bedrock to AmazonBedrock per review request
- Use :amazon_bedrock provider ID to match upstream models.dev
- Move AWS Event Stream parser under AmazonBedrock namespace
- Remove debug.log file
- Revert changes to priv/models_dev/amazon-bedrock.json (use upstream version)
- Fix Context API usage in tests (use Context.user/system helpers)
- Fix StreamChunk type expectations (:content and :meta)
- Add @dialyzer :no_match suppression for DSL macro issues
- Fix system message extraction in Anthropic formatter
- Update all test files to handle provider renaming

Addresses all of Mike's review comments:
1. Removed debug.log
2. Moved AWS Event Stream under AmazonBedrock namespace
3. No modifications to models_dev JSON files
4. Renamed Bedrock to AmazonBedrock throughout

All 959 tests passing, live streaming verified with AWS credentials.
