Fix test failures for :bedrock provider ID

- Update test files to handle amazon-bedrock.json mapping to :bedrock
- Fix Context API usage in tests (use Context.user/system helpers)
- Fix StreamChunk type expectations (:content and :meta)
- Remove debug.log file
- Move AWS Event Stream parser under Bedrock namespace
- Add @dialyzer :no_match suppression for DSL macro issues
- Fix system message extraction in Anthropic formatter

All 959 tests now passing with 0 failures.
