Add ClickHouse to README, record variable support

Add ClickHouse to `README.md` and reformat the badge table at the top of
the file to include ClickHouse and the MariaDB tests broken out in
16d3838 by adding a new column.

Replace the Pod MySQL-copied Pod in `clickhouse.pm` to document the
supported version, URL parameters, and methods.

Add ClickHouse to the various manual pages:

*   Update all documentation links to pages for each engine that
    supports variables so that each is on a single line, and add
    ClickHouse to the list.
*   Add `clickhouse` to the list of supported engines in
    `sqitch-add.pod`, `sqitch-engine.pod`, `sqitch-init.pod`,
    `sqitch-target.pod`, and `sqitch.pod`
*   Note the ClickHouse authentication options in
    `sqitch-authentication.pod`
*   Document the ClickHouse environment variables in
    `sqitch-environment.pod`
*   Add `<engine.clickhouse.registry` to the list of registry
    configurations in `sqitch-config.pod`
