Fix when listeners get notified about entry to / exit from lexer rules in C++ (#330)

The Python runtime already properly handles the notification of
listeners about token fragments, i.e., listeners see the name of
the fragment on entry / exit. In the C++ runtime, the listeners
saw the name of the enclosing lexer rule. This commit fixes this
issue.
