refactor: change refresh parameter from boolean to string

- Allow arbitrary string values for cache busting instead of just true/false
- Update controller to accept string with empty default instead of boolean
- Update OpenAPI documentation with string type and example
- Modify all interfaces and implementations across the stack
- Update cache key logic to use string value directly
- Update tests to verify string parameter handling

This enables repeated cache invalidation by using different string values
(timestamps, UUIDs, etc.) instead of being limited to true/false.
