fix: restore ping endpoint functionality and add test coverage

- Fixed router initialization to properly support middleware
- Both /ping and /api/v1/ping endpoints now work correctly
- Added test coverage for ping endpoints to prevent future regressions
- Updated documentation to reflect both endpoints are supported

The issue was with router initialization - needed to return the router
instance for middlewares to function properly.
