fix: PIN input height consistency and HTMX error handling

- Fix PIN input height to match other inputs (60px) by adjusting font-size from var(--text-lg) to var(--text-base) and padding from 13px to var(--spacing-md)
- Fix HTMX validation error targeting from #form-errors to #form-card for proper error display
- Add HTMX event handlers to clear expiration validation errors when user changes field values
