XML entities fix for plutil -lint happiness (numeric unless XML-specific) (#36)

* Convert only XML-specific entities to equivalent on output

Non-specific ones are encoded numerically (else plutil complains)

* Output dict keys sorted (so results are consistent)

* Remove entity processing for Base64 text

There are not escapable characters in Base64 text.
