refactor: improve code quality in main.go

- Refactor initializeProviders() from 91 lines to 50 lines
  - Extract provider configurations into getStandardProviderConfigs()
  - Move providerConfig struct to package level
- Refactor executePrompt() from 51 lines to 40 lines
  - Extract mix mode processing into processMixMode()
- Fix ignored error handling for os.Stdin.Stat()
- Remove misleading hardcoded temperature values for Anthropic and Google providers
  - These providers don't use temperature parameter in their API calls
- Remove temporary smells.md analysis file

All tests pass and no linting issues.
