Commit Message

Coveralls, refactored CI files, tox, setup.py.

Refactoring CI config files, tox.ini, and setup.py. Same layout as my
other projects.

Switching from codecov to Coveralls. Using appveyor-artifacts to
overcome Coveralls' single-ci limitation.

No longer generating a requirements-test.txt. Putting all test
requirements in tox.ini.

Removing pylint testenv for now. Wasn't being used anyway.
