Refactor indexer_spec

- Pull out base case `let` statements.  That allows many of the base
  contexts to flatten into single examples.
- added TODO at anti-pattern each loop around specs
- Rubocop-initiated cleanups (e.g. not_to vs. to_not)
- For solrdoc, check the stronger does not include (instead of is blank)
  where possible.  This *should* be the preferred behavior for all the
  indexing methods: do not add empty arrays to the solrdoc hash.
