Fix #304: Don't send reasoning_effort parameter when set to 'none' (#305)

- Only add reasoning_effort to OpenAI request when not empty and not 'none'
- This prevents 400 errors with non-reasoning models like gpt-4o-mini
- Update test to reflect that 'none' value should not be sent in request
