Add --use-regex/-r and --insensitive/-i options to count_phrases

With these additions, the default behavior has been switched to
case-sensitive for the count_phrases analyzer. This is a
backwards-incompatible change and will necessitate a semver-major
release, but makes for a more intuitive API.
