* feat: add adType parameter to support segment insertion without cue tags

- Add adType parameter to insertAdAt() with ad (default) and segment options
- Segments insert content with discontinuity tags only, no cue-out/cue-in
- Ads maintain existing behavior with full cue tag support
- Update tests to verify mixed ad/segment insertion scenarios
- Maintain backward compatibility with default ad type

* chore: bump node-m3u8 to version 0.5.9

* update readme
