Commit Message

major refactoring:
- removed MessageElement and map the value directly
- replaced Section with CodecContext (a thread local approach)
- make codec attributes immutable
- remove building of index path in CodecException and just use CodecContext
- cleanup XmlConfig attribute methods and removed optional attributes
- added fully-qualified-class-name to identify which attribute should translated as a class
- used ElementCodec for logging element details and raw value
