internal: remove unnecessary field from AddColumn and ModifyColumn

This commit removes an unused Table reference field from both the AddColumn
and ModifyColumn structs. Clause-level methods don't need a reference to the
full Table in order to emit clause SQL or determine diff safety; and callsites
for those methods already have the full Table available anyway. Removing
unused fields will simplify future support for diff serializability (e.g. JSON
diff output).

No changes to Skeema functionality in this commit.
