Length validation (#88)

* Introduce SizeAtLeast, SizeAtMost, and SizeWithin validators. 

These operate on a numeric field (e.g., UBInt8), raising a
ValidationError if given a value that isn't within their specified
bounds.

* TLSPrefixedArray now accepts a length_validator.

* tls._constructs now uses SizeAtLeast, SizeAtMost, and SizeWithin.

* Includes some miscellaneous docstring improvements as well.

* Addresses @ashfall's code review in pyca/tls#88.  Thanks!
