fix: Track discovered nodes to avoid "rediscovery" (#168)

This violated the contract of `iteration` which says that the `nodes`
passed in are *new* (not already received). In turn, this was breaking
some of the candidate tracking in MMR since duplicates were arising.

This closes #167.
