fix: prevent proposal/confirmation of nonces lower than the Safe (#2408)

We refine transactions of the API, proposals and confirmations. Currently, the API refinement is only for the queue.

This adds similar checks (throwing an error) if incoming proposals or confirmations are for transactions with a nonce lower than the Safe and/or for those which are executed:

- Add and throw new error for appropriate proposals/confirmations
- Add/update tests accordingly
