In 1173 create reconcile report (#160)

Why these changes are being introduced:
* Share a detailed report summarizing results from 'reconcile' runs

How this addresses that need:
* Create HTML and plain-text templates for 'reconcile' reports
* Update SimpleCSV and OpenCourseWare workflow to record events for
'reconcile' reports
* Define custom warnings for reconcile failures
* Rework 'reconcile_bitstreams_and_metadata' to return boolean values
* Raise 'ReconcileError' explicitly at the CLI level
* Set up email attachments as CSV files 
* Update logging
* Update unit tests
