The README.markdown contained two references to the disabled xen-api
mailman list, see:
https://lists.xenproject.org/mailman/listinfo/xen-api.disabled

- Replace them with the currently used issues and discussions pages on
GitHub.
- While at it, mention the DCO as requirement for merging patches.
