Enhancement unity.host._get_host_lun to get specified lun quickly (#5)

Enhance the UnityLun filter.  Details:

* Fix UnityResourceList.get will return fake object issue.
* Fix system.get_iscsi_portal since there is no 'name' property at all
* Make UnityNameNotUniqueError can return the found objects
* Directly using UnityHostLunList filter with host.id and lun.id
