Write/enqueue only active commands #262

CommandHandler writes and queues only active (non-cancelled) commands and command batches. Command batches are inspected for Commands were previously not queued but written which caused the connection to get out of sync. CommandEncoder no longer checks for cancelled commands as this would cause the connection to get out of sync. Retriggering of commands in cluster node connection uses the cancelled commands check to optimize its writes.
