pipeline refactor (#205)

* work on high-level pipeline running.

* Work on rewriting desi_pipe and making many small changes to get things working together.  desi_pipe_status still needs work to be useful.

* Work on pipeline status reporting.

* Remove old pipeline code.

* Many small fixes.  Still debugging output stream hangs in logging.

* Small bug fixes and cleanup of debugging statements.

* If zfind is run with the default number of processes, first check whether we are running under slurm and use the appropriate environment variable to get the max number of processes to use.

* Change zfind concurrency in generated slurm scripts to use 8 master processes per node, rather than one.

* Disable read-and-broadcast of the PSF until all PSF classes can be pickled.

* Change default number of zfind workers

* Include stacktrace along with yaml inputs for failed tasks.

* fix bugs in error dumping; add timestamps

* Ensure that we create all output exposure directories when setting up a production.

* When planning and creating a production, compute the total number of bricks.  This is used when creating the slurm scripts to know how large a job to run.

* mimic X11 for Travis tests
