Implement deferred Promise, cleanup.

Implement a deferred Promise, which suspends resolution until its value
is actually requested, and resolves in a requesting goroutine instead of
its own.

Rework Promise as a struct instead of an interface, since its interface
type was never intended to be acutally implemented by anything other
than the Promise.
